View Medical Record via SystmOnline

Patients can view their medical record using SystmOnline, to view your medical record via SystmOnline click the View Medical Record link on the Patient Record page.

View Medical Record via the NHS App

Patients can view their medical record using the NHS App, by clicking the 'Get Started' button below. Once you've logged in/registered you should notice 5 icons at the bottom, you need to click 'Your Health Icon' then click 'GP Health Record'.

How long does it take to request access to my medical record?

We aim to respond to requests within three weeks. If you do not hear back within this time write again or call to request an update. If you have not heard anything after 40 days you can make a formal complaint.

How do I access my medical record?

If you want to access your medical record and you haven't already signed up for online services you will need to fill out an online access form that can be printed off and brought into the surgery with valid proof of address and photo identification *please note if you do not have a printer we can provide you an online access form to fill out at the surgery. Once you've filled out an online access form you will be given an account ID, account linkage passphrase and an organisation code. Once you have these details you can sign up via SystmOnline by clicking sign up at the top of the page.

How much of my medical record history can I request?

You can access your medical record history for the duration you have been with us at Lindley Village Surgery, if you was previously registered with another practice you will have to submit another medical record access request with them.
