You can visit the surgery during working hours to get your account ID, account linkage passphrase and an organisation code from reception. Once acquired you can sign up below.

SystmOnline

SystmOnline allows you to book and cancel appointments, request access to your medical record and request repeat medication, if you'd like online access but haven't already signed up you will need to fill out an online access form at the surgery. Once you've filled out an online access form you will be given an account ID, account linkage passphrase and an organisation code. If you have these details you can sign up by clicking the button below.

It won’t allow me to login.  I have tried to reset my password but it says " unable to perform this request" What should I do?

It is likely that we do not have a verified email in your patient record, so we do not have a way to send you a verification link. You will need to contact us to reset your password and provide a verified email. We advise that you bring identification such as a passport or driving license into the surgery as this will be required  in order to rest your password.

I have forgotten my user name and I am unable to login to my account for SystmOnline. What should I do?

You will need to contact us to get your username if you've forgotten your username. We advise that you bring identification into the surgery such as a passport or driving license as this will be required to regain access to your account.

How do I view my patient record?

If you have been granted access to your full clinical record or your detail coded record view you can find this by clicking the Patient Record link on the Patient Record page. If you have not already requested access, you can request access to this service by clicking Manage Online Services from the Online Account Management page. Click the Patient Record link on the Patient Record page after a week or so to see whether access has been granted. If it has not, either wait a few days longer or contact us to get an update.

How do I change my appointed pharmacy?

If you'd like to change the pharmacy you collect your prescriptions from click the Change Pharmacy link on the Medication page then click Choose beside the pharmacy you want to use.
